Clinical Agency Requirements

Information for Compliance with Health and Agency Mandates

Please print and fill out entire form and submit to Sandra Mann in the College of Health and Human Services Advising Center (CHHS 103). There may be additional Compliance Requirements - (All of these may not be required by your agency, or additional requirements may be needed. Check with your program coordinator.)

Clinical agencies require students to comply with all health and training requirements of any clinical placement agency. We have listed many resources here for your convenience.

It is a SON policy that ALL students who have a clinical placement MUST complete a Criminal Background Check and Drug Testing using only university vendors as listed here. Please complete the Bloodborne Pathogen Tutorial and Test as well as the Hepatitis B Delination Form and submit to Sandra Mann in the College of Health and Human Services Advising Center (CHHS 103).

It is a SON policy that all Graduate Students who have a clinical placement MUST carry liability insurance. These three companies who offer Liability Insurance to students. Please submit a copy of the insurance certificate showing proof of coverage to Sandra Mann in the College of Health and Human Services Advising Center (CHHS 103).

Students assigned to the following agencies must complete cited Orientation Packets. 
 
Graduate student orientation packets: Please complete and print pages such as; module posttest answer sheets, data forms, and forms that require signatures and submit to the administrative assistant in CHHS 418 prior to the beginning of scheduled clinical/internship/preceptorship.
 
Undergraduate student orientation packets: Please complete and print pages such as; module posttest answer sheets, data forms, and forms that require signatures and take them to your clinical faculty the first day of clinical.
